Request Past Reports from Legacy System

This article covers how an instructor can request PDF copies of past reports from legacy eval system (WDYT).
You only need to submit one form even if requesting mulitple reports.
Instructions
PDF versions of all Boise State University Instructor Reports from WDYT have been saved and archived. We can share these reports with you upon request.
Please fill out the legacy system (WDYT) past reports request form.
Turnaround time: Within 1 week (7 days).
Submit just one form. You will receive all the past evaluations we have archived for the term(s) you are requesting as long as the minimum enrollment thresholds (Policy 4300 6.1) have been met.
The term, department, subject, course number, etc. fields are used to help ensure we are pulling the correct reports. However, we do not need an exhaustive list. The most important information are the terms you need and your username, and particularly any past usernames if you have had a name change.
