This article covers how an instructor can request PDF copies of past reports from legacy eval system (WDYT).

You only need to submit one form even if requesting mulitple reports.

Instructions

PDF versions of all Boise State University Instructor Reports from WDYT have been saved and archived. We can share these reports with you upon request.

  • Turnaround time: Within 1 week (7 days).

  • Please only submit the form one timeSubmit just one form. You will receive all the past evaluations we have archived for the term(s) you are requesting.

  • The term, department, subject, course number etc. fields are used to help ensure we are pulling the correct reports. However, we do not need an exhaustive list. The most important information is the instructor’s terms you need and your username, and particularly any past usernames if the instructor has you have had a name change.
